(b) The American Wyle Co. metal lathe was not equipped with a shield to prevent injuries to employees from flying metal chips.    (c) The red LeBlond metal lathe was not equipped with a shield to prevent injuries to employees from flying metal chips.    (d) The Dake hydraulic press has no guarding to prevent injuries to employees from flying objects.    (e) In-running nip points and rotating parts below the table of the Marvel Mark II vertical bandsaw was not fully guarded exposing employees to serious injuries.

Power House Machine Shop-  (b) The Iron Worker "Metal Muncher" foot treadle's electrical cord was not attached to prevent strain or tension on the conductors causing the employees to be exposed to electrical shocks.       Power House 4th floor main unit 5-  (c) The portable forklift charger, SN# 7955s plug was not connected to the plug to prevent tension from being placed on the conductors, exposing employees to electrical shocks.
