29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening shall be constantly attended by someone or shall be protected on all exposed sides by removable standard railings.    (a) The "E- crane's" ladder way access to the operator's station was not guarded exposing employees to falls of approx. 15 feet.

(a) The ladder rungs of the "E- crane's" ladder way access to the operator's station exposed worker to falls from the ladder because of deteriorating condition of the non-slip tape.

29 CFR 1910.184(c)(7): Sling(s) were not padded or protected from sharp edges of their loads:     Gate Repair Area-  (a)  Round synthetic slings were attached to steel beams using a choker configuration without padding while employees were using these slings as a part of their fall protection system. This hazardous condition exposed employees to 4 foot and greater falls in the event of these slings failing. The employees were performing cutting/welding repairs of gates.    Gate Repair Area-  (b) Round synthetic slings were attached to steel beams using a choker configuration without padding while being used to suspend chain falls that were being used to raise steel plates and other materials, exposing employees to being struck- by equipment and materials.     Note: Abatement certification and supporting documentation are required for these items.
