At the time of the inspection the lift truck was located in the north yard of the Zidell facility at 3121 SW Moody Avenue in Portland, Oregon.    (b) On or about November 7, 2012 employees were potentially exposed to serious injury from the use of the green Mitsubishi lift truck, serial #F15B-00132, model FD100, that had a cut on the front right tire and a damaged rim on the front left tire. At the time of the inspection the lift truck was located in the north yard of the Zidell facility at 3121 SW Moody Avenue in Portland, Oregon.
