29 CFR 1910.178(p)(1):     Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating conditions:    (a) On or about November 7, 2012 employees were potentially exposed to serious injury from the use of the green Mitsubishi lift truck, serial #F15B-00132, model FD100 that was not equipped with a seat belt. At the time of the inspection the lift truck was located in the north yard of the Zidell facility at 3121 SW Moody Avenue in Portland, Oregon.    (b) On or about November 7, 2012 employees were potentially exposed to serious injury from the use of the green Mitsubishi lift truck, serial #F15B-00132, model FD100, that had a cut on the front right tire and a damaged rim on the front left tire. At the time of the inspection the lift truck was located in the north yard of the Zidell facility at 3121 SW Moody Avenue in Portland, Oregon.

29 CFR 1910.179(n)(3)(v):     Employee(s) were on the load or hook while hoisting, lowering or traveling was performed:    (a) Employees were potentially exposed to serious injury from the use of personnel platforms that do not meet design specifications to hoist and lower employees from the shipyard "whirly" cranes. Neither platform had permanent markings to to indicate the platform's weight and its rated load capacity or maximum intended load. Located in the erection area of the facility at 3121 SW Moody in Portland, OR.

29 CFR 1915.132(d):     Worn and or frayed electric cables were not taken out of service:    (a) Employees were potentially exposed to serious injury from the use of the Dewalt portable grinder, number 2010 21-47 with a damaged flexible cord. The grinder was in use on the deck of the barge under construction at 3121 SW Moody in Portland, Oregon.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    (a) Employees were potentially exposed to serious injury from a flexible cord in use on the barge under construction that had melted onto a piece of metal and was laying on the metal walkway. The hazard was located inside the barge under construction at 3121 SW Moody in Portland, Oregon.
