29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) New Mess Hall, West roof area: On November 7, 2012, and times prior thereto, employees were working on a 6 inch in 12 inch pitch roof installing metal roofing using rope grabs that were rigged so they would allow a fall of approximately 11 feet and contact with the ground if they fell. Note: Abatement Certification is required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: a) New Mess Hall roof area: On November 7, 2012, and times prior thereto, employees that were installing new roofing on a 6 inch in 12 inch sloped roof were not trained in the proper use of the rope grab and harness system in that they were not adjusting the slack in the rope grabs to prevent them falling 11 feet and hitting the ground. Note: Abatement Certification is required for this item
