The employer had not evaluated since 2005 the performance of the employee operating the Gehl RT forklift, Model Number 553.

29 CFR 1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970: a) Butler Area Sewage Authority jobsite, 100 Litman Road Butler, PA: On or before 11/14/12, two (2) 40 BC rated portable fire extinguishers located at the masonry mixer were not inspected since 2006. One of the portable fire extinguishers, Model 5MA-6H, was completely discharged. Etzel Masonry Inc., was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 29 CFR 1926.150(c)(1)(vii) which was contained in OSHA Inspection number 314107384, Citation number 1, Item number 4b, issued on 08/25/10, with respect to a workplace located at A.W. Beattie Career Center yard area, 9600 Babcock Boulevard, Allison Park, PA.
