29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Ithaca Gun Co., Upper Sandusky, Ohio:  On or about November 20, 2012, the employer failed to develop and implement a lockout program, with procedures, training and audits to protect employees from injuries while they engaged in servicing and maintenance activities.  Employees were exposed while performing tasks such as, but not limited to job setups and tooling changes on various CNC machining equipment, exposing them to laceration, crushing and amputation hazards.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Ithaca Gun Co., Upper Sandusky, Ohio:  On or about November 20, 2012, employees operating the #1 FANUC ROBODRILL Mate CNC machining center were exposed to injuries from flying particles and rotating parts, in that the interlock on the sliding door guard was bypassed.  Employees were able to access the hazard area while the machine was in operation.    b) Ithaca Gun Co., Upper Sandusky, Ohio:  On or about November 20, 2012, employees operating the #2 FANUC ROBODRILL Mate CNC machining center were exposed to injuries from flying particles and rotating parts, in that the interlock on the sliding door guard was bypassed.  Employees were able to access the hazard area while the machine was in operation.     c) Ithaca Gun Co., Upper Sandusky, Ohio:  On or about November 20, 2012, employees operating the #3 FANUC ROBODRILL Mate CNC machining center were exposed to injuries from flying particles and rotating parts, in that the interlock on the sliding door guard was bypassed.  Employees were able to access the hazard area while the machine was in operation.      d) Ithaca Gun Co., Upper Sandusky, Ohio:  On or about November 20, 2012, employees operating the Baldor Electric Co. manufactured buffer (Cat. No. 407B, Serial #X0707277021) were exposed to rotating parts, in that the buffer's shaft and flange nut were unguarded, exposing employees to caught-by hazards.     e) Ithaca Gun Co., Upper Sandusky, Ohio:  On or about November 20, 2012, employees operating the Baldor Electric Co. manufactured buffer (Cat. No. 410B, Serial #X0801157014) were exposed to rotating parts, in that the buffer's shaft and flange nut were unguarded, exposing employees to caught-by hazards.
