On or about 11/15/2012 at 11350Johns Creek Parkway Duluth, GA; employees were exposed to trip hazards in the call center where the data and power cables for the computers and phone lines were hanging from the workstations to employees' feet.

On or about 11/15/2012 at 11350 Jones Creek Parkway Duluth, GA; employees performing maintenance tasks on equipment that has multiple energy sources are exposed to struck-by and caught-in hazards where the specific procedures did not include:    (A) The step of verifying the machine is off by attempting to start after terminating power.    (B) The step of ensuring that all stored energy is effectively released.
