29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) North East Corner of the building - Employees were working from the platform of an Clark aerial lift installing ornametal steel at the northeast corner of the building. The foreman who was working from the platform was not tied off to the basket, on or about October 12, 2012.

29 CFR 1926.303(c)(3): Portable abrasive wheel(s) used for external grinding were not provided with safety guards (protection hoods): (a) North East Corner of the building - An employee was using a Metabo right angle grinder that had a four inch abrasive wheel attached to the grinder. The employee was grinding on the outside of a piece of ornament steel. The right angle grinder was not equipped with a guard, on or about October 12, 2012.
