29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Zenith Systems, LLC, Clyde, Ohio: On or about November 16, 2012, at the Commodore Perry Travel Plaza, Eastbound I80/90, Clyde, Ohio, the employer did not ensure that an employee was protected from falls while installing new data cable on an elevated working surface. The employee was exposed to a fall of approximately 12 feet to the ground below, which could result in permanent disability or death.
