Atlas Bolt & Screw Company LLC located in Ashland, Ohio:  On or about October 25, 2012, the employer did not ensure an annual audit was conducted of the employers lockout procedures to ensure the procedures were being properly implemented.

Atlas Blolt & Screw Company LLC located in Ashland, Ohio:  On or about October 25, 2012, the employer did not ensure employees operating the #1 closure press were trained as authorized employees under the company's lockout program.

Atlas Bolt & Screw Company LLC located in Ashland, Ohio:  On or about October 25, 2012, the employer did not ensure employees unjamming the #1 closure press deenergized the equipment prior to placing their hands under the die to retrieve the jammed part.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. Atlas Bolt & Screw Company LLC located in Ashland, Ohio:  On October 25, 2012, the employer did not ensure the Atom Model 5677/2 CEFM hydraulic press located in the closure department, was properly guarded.  The guards were easily removable from the side of the press and the removal of the guard did not stop the downward motion of the die.    Atlas Bolt & Screw Company LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 129 CFR 1910.212(a)(1), which was contained in OSHA Inspection Number 313783912, citation number 1,  item number 1 and was affirmed as a final order on October 12, 2010, with respect to a workplace located at 1628 Troy Road, Ashland, Ohio.
