29 CFR 1910.269(a)(2)(i): Employees were not trained in nor were they familiar with the safety-related work practices, safety procedures, and other safety requirements that pertained to their respective job assignments: Powerhouse 5th floor, Bay 1- (a) Employees involved in the switching evolution on or about July 17, 2012 were exposed to serious injuries when an Arc Flash incident occurred. The switching order was incorrect and allowed a switch to be in an energized state and under a load as a trainee was operating a crank to open the disconnect when 201 kW/10 amps of energy was released. Note: Abatement certification and supporting documentation are required for this item.
