(a) Construction site of the Ventura Campus of Rancho Solano School (Salt River Pima-Maricopa Indian Community), 9180 E. Via De Ventura, Scottsdale, AZ : On or about 16 November 2012, the employer erected and used a tubular welded frame scaffold supporting a working area platform approximately 9 feet above the ground level. A cross bracing of one section was not connected to and secured to the scaffold end frame, thus exposing employees to hazards from an unstable scaffold.
