29 CFR 1910.134(c)(1):   A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use.    MANUFACTURING SHOP (OLD BUILDING):  On or about November 15, 2012, the employer's Respirator Program did not:     - Address requirements for medical evaluations prior to respirator use     - Outline the procedures for proper use of respirators     - Provide training protocols on respiratory hazards     - Provide training procedures for proper use as well as donning and doffing of respirators     - Incorporate procedures for regular evaluation of the effectiveness of the Respirator Program  Employees are exposed to chemicals including, but not limited to, styrene and methyl methacrylate.

29 CFR 1910.134(c)(3):   The employer did not designate a program administrator who was qualified by appropriate training or experience to administer or oversee the respiratory protection program and to conduct the required evaluations of program effectiveness:    On or about November 15, 2012, during the initial inspection, the employer provided a copy of the company's Respirator Program which referenced activities to be performed by the Safety Administrator but no individual had been identified.  Employees are exposed to chemicals including, but not limited to, styrene and methyl methacrylate.

On or about November 15, 2012, the employer's Respirator Protection Program did not (1) identify or evaluate the respiratory hazards, (2) provide a reasonable estimate of employee exposures, or (3) identify the chemical state and physical form of the contaminants. On or about November 15, 2012, the employer's Respiratory Protection Program requires that employees consult their own physician if they "feel that there is a question about (their) health as it relates to the use of respiratory equipment".
