On or about November 30, 2011, located outside of the spray booth, several windows and doors were laid on the one side of spray booth and an old copy machine was stored at the other side of the spray booth. A clear space of at least 3 feet was not maintained on both sides of the spray booth and kept clear of storage to allow for ready accessibility for cleaning and removal of any fire hazards.

On or about November 15, 2012, the employer provided employees 3M 8210 Particulate Respirators and 3M tekk Sanding and Fiberglass Respirators for voluntary use while operating cutting and sanding activities in Cutting Area and Sanding Area. Employees had not received medical evaluations to determine their ability to use a respirator. The employer had not established nor implemented a written respirator protection program.
