29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. Mixing Room- on or about November 19, 2012, the mixing room was not kept clean from combustible dust accumulations. Dust accumulated on shelves, walls, piping and production equipment.

29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: Lunch room- On or about November 19, 2012, wipe samples were obtained from lunch room tables. Analytical results indicated the presence of toxic metals, as follows: 1.) Right lunch room table: 6.845 ug (micrograms) Neodymium; and 2.) Left lunch room table: 1.64 ug (micrograms) Neodymium.
