29 CFR 1910.1048(d)(2)(ii): The initial monitoring process was not repeated each time there was a change in production, equipment, process, personnel, or control measures which could have resulted in new or additional exposure to formaldehyde: On or about November 13, 2012 and at times prior thereto, the employer had not conducted initial monitoring when there was a change in the process involving formaldehyde. A new batch mixer was installed in Line 2 during the first week of October 2012 and the employer did not conduct full shift sampling until on or about November 21, 2012, exposing employees to the hazards of formaldehyde.
