29 CFR 1910.101(b): Compressed gas cylinders were not handled, stored and utilized in accordance with Compressed Gas Association Pamphlet P-1-1965: Refer to section 3.3.5 of Pamphlet P-1: Cylinders were stored at temperatures above 125 degrees F or near radiators or other sources of heat: Chlorine cylinders were stored in a heated shed near a space heater which overheated them resulting in a chlorine release when the fusible plugs melted.

29 CFR 1910.119(e)(1): The process hazard analysis was not appropriate to the complexity of the process and did not identify, evaluate, and address the control of the hazards involved in the process: The process hazard analysis which was done for the titanium dioxide process did not include all nodes which were part of the process and could therefore impact the safety of the process. Nodes covering, including but not limited to, chlorination of raw materials, purification of raw materials, wastewater treatment and finishing were not evaluated to determine what hazards they presented and evaluate if measures in place were sufficient to address these hazards and determine if any additional controls should be implemented.
