29 CFR 1910.119(j)(6)(i): The employer did not assure that equipment as it is fabricated is suitable for the process application for which they will be used: Refer to American Society of Mechanical Engineers (ASME), PCC-2 Repair of Pressure Equipment and Piping 2011, article 2.11 section 3.1 which requires a determination of the causes of damage prior to application of a weld overlay on a pressure vessel. The chlorine vaporizer EA-102, NB #10382, showed galvanic corrosion during an inspection made when the vessel was removed from service. In an attempt to reduce the corrosion which resulted from an interaction between the Monel tubes and the carbon steel tube sheet, a decision was made to place a cladding on the tube sheet. This cladding acted as a nexus for pitting which subsequently caused failure of the tube sheet resulting in chlorine leakage into the condensate water. No evaluation was made to determine what effect installing the cladding would have on the rate of corrosion of the tube sheet prior to ordering the repair to be made.

29 CFR 1910.119(l)(2)(i): The written procedures to manage changes did not assure that the technical basis for the proposed change was addressed prior to any change: The chlorine vaporizer EA-102, NB #10382, showed galvanic corrosion during an inspection made when the vessel was removed from service. In an attempt to reduce the corrosion which resulted from an interaction between the Monel tubes and the carbon steel tube sheet, a decision was made to place a cladding on the tube sheet.
