At the site, employees were exposed to possible cave-ins, in that, a ladder or other means of egress were not provided while in a trench measured 8 feet wide by 12 feet long and 7 feet 10 inches deep.

At the site, employees were exposed to possible struck by hazard, in that, no protective system was provided to protect employees from cave-ins while working in a trench 8 feet wide, 12 feet long, and 7 feet 10 inches deep, with vertical walls in type "C" soil.
