29 CFR 1910.25(d)(2)(xv): No ladder should be used to gain access to a roof unless the top of the ladder extends at least 3 feet above the point of support, at eave, gutter, or roofline: On or about November 21, 2012, employer did not enforce the proper use of a ladder. The ladder was not extended at least 3 feet over the roofline. This condition exposed employee to potential fall hazards.
