29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about March 3, 2017, employee operating an boom truck with bucket at a commercial site was removing a sign and was not wearing any type of fall protection. This condition exposed employee to falls of 10 feet to the ground level. Alavista Signs was previously cited for a violation of this Occupational Safety and Health Standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1051662, citation number 1, item number 2 and was affirmed as a final order on May 30, 2015 with respect to a workplace located at 7101 North Mesa Suite B, El Paso, TX.

On or about March 3, 2017, the employer did not ensure that employees with fall protection training necessary for hazard recognition retained the basic skill and understanding for safe work practice. This condition exposed employees to falls exceeding 10 feet.
