It was found in use to secure parts in a locker.

29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not accessible for immediate emergency use:    In the Fabrication Department Supply Room: The employer did not ensure that employees had an unobstructed path to a safety shower and eyewash. Employees use bleach (corrosive) to wash white garments such as gloves and aprons that are blood soaked. A safety shower and eyewash are located in the room however they were blocked by a metal storage rack and plastic barrels containing liquid bleach.

29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    In the kill floor maintenance room: The employer provided a Dayton 6" Bench Grinder (model #  ZLKR6) for use by the employees. The employer did not ensure that the tongue guard was adjusted to a space that would not exceed one fourth of an inch.

29 CFR 1910.23(c) (1):  Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:    At Nebraska Beef the crossover platform crossing over the Boxing Machine in the Scale Room: The railing consisted of a top rail at 42 inches however it did not have an intermediate railing at 21 inches as required for a standard railing in accordance with the OSHA standard.      Nebraska Beef, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.23(c)(1) , which was contained in  OSHA inspection number 314058876, Citation #1, Item #1, and became a final order on 3-31-2011.
