29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. On or about November 19, 2024, it was documented that employees engaged in material handling in the shipping department of Nebraska Beef Ltd located at 4501 South 36th Street, Omaha, Nebraska were exposed to struck-by hazards in that the employer did not ensure newly hired powered industrial truck operators completed practical training and evaluation prior to unsupervised use of Crown ISS reach stacker and/or Crown PE 4500 Series motorized pallet jack.
