29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.    Location - Adams Street & Constitution Avenue, Revere, MA:  On or about 11/21/2012, an employee working on a roof that was greater than six feet above a lower level was not protected from falling.     Meadows Construction Company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection numbers 315871970, 315486738, Citation Number 01, Item Numbers, 003, 001, Issued on 12/09/2011, 6/02/2011 with a final order date of 1/30/2012, 6/27/2011 with respect to workplaces located at Spring Court Extension, Woburn, MA and 134 McKay Street, Beverly, MA;
