Main St., N. Brookfield, MA: On or about June 22, 2015 a 10 inch portable DeWalt brand table saw was used without any guard present on the saw.

Main St., N. Brookfield, MA: On or about June 22, 2015 the guardrails placed around a floor opening (antique bell pull rope opening) were not capable of withstanding, without failure, a force of at least 200 applied in an outward or downward direction at any point along the top edge of that railing.

Brookfield, MA: On or about June 22, 2015;     a) a portable GFCI device was powered through an extension cord  b) metallic receptacle boxes were joined together to form a freestanding electrical extension cord receptacle    neither of which were installed and/or used  in accordance with instructions included in the listing, labeling and/or certification for the device.    Meadows Construction Company, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.403 (B) 2 which was contained in OSHA inspection number 315871970, citation number 1 item number 1a and was affirmed as a final order on 1/30/2012 , with respect to a workplace located at Spring Court Extension, Woburn, MA.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:    a) 125 N. Main St., N. Brookfield, MA: On or about June 22, 2015 an employee was working in front of and leaning out of the 3rd floor window opening at this address, where the outside bottom edge of the wall opening was more than 30 feet above the concrete sidewalk below and the inside bottom edge of the wall opening was 1.4 feet (17 inches) above the walking/working surface.      Meadows Construction Company, LLC  was previously cited for a repeat violation of an equivalent standard, 29 CFR 1926.501(b) 13, which was contained in OSHA inspection number 749241, citation number 1, item number 1 and was affirmed as a final order on October 17, 2013, with respect to a workplace located at Adams St. and Constitution Ave., Revere, MA.
