Haskell Avenue, Dallas, TX 75204; where at least one (1) employee worked from an aerial lift basket and was not wearing a body belt with lanyard attached to the boom or basket.        COMPOSITE PANEL TECHNOLOGY SOUTH, INC. WAS PRECVIOUSLY CITED FOR A VIOLATION OF 29 CFR 1926.453(B)(2)(V) OR IT'S EQUVALENT, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312118623, CITATION NUMBER 01, ITEM 02, AND WAS AFFIRMED AS FINAL ORDER ON MARCH 11, 2009,  WITH RESPECT TO A WORKPLACE LOCATED AT 1000 E.
