29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. The employees were observed working on a flat top roof 20 feet above the ground without fall protection. The roof had a 2 foot parapet. All employees controlled on the site were on the roof performing removal and reinstallation of insillation.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. The employer removed the grounding pin and allowed for the use of a defective extention cord. The outlet at the aged facility was not able to support a 3 wire service with ground so the employer removed the grounding pin in order to utilize the electrical service available.
