29 CFR 1926.150(c)(1)(vi):A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas are being used on the jobsite: (a) 1020-1024 Dwight St Springfield, MA: On or about November 23, 2012, employees replacing the roof at this address were using a gas powered air compressor to power their nail guns, and in addition had multiple containers of gas (3 gal & 5 gal) to fuel the compressor with no fire extinguisher available at the site.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system: (a) 1020-1024 Dwight St Springfield, MA: An employee was observed stripping the roof on this multifamily residential building more than 17 ft. above the ground below without any fall protection as required by this subpart: Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
