29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the forklift: a) Delagrange Construction at the worksite located at 222 W. High St. in Hicksville, OH: The employer did not assure that a personnel platform, used to lift employees and materials to the roof, was secured to the lifting carriage of the JLG Lull (model 644E-42) rough terrain fork lift. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Delagrange Construction at the worksite located at 222 W. High St. in Hicksville, OH: Employees working from a scaffold that was supported by a JLG Lull (model 644E-42) rough terrain fork lift were exposed to falls of approximately 12 feet. The employer did not assure that the working platform was equipped with a standard guardrail on all sides. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
