(a)Eagle Eye Construction, LLC @ 2243 W. 32nd Ave., Denver, CO 80211: On and before November 28th, 2012, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in framing activities while working at the unprotected edges of the third story on a multi-family home under construction. Employees were not utilizing a fall protective system. Employees were exposed to fall hazard of approximately 20'.

29 CFR 1926.1053(b)(1): A portable ladder was used for access to an upper landing, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing, and was not secured: (a)Eagle Eye Construction, LLC @ 2243 W. 32nd Ave., Denver, CO 80211: On and before November 28th, 2013, the exposing employer did not ensure that a portable step-ladder, used to access the third level of a multi-family home under construction, had side rails extending 3 feet above the upper landing, or was secured. The condition exposed framing employees to uncontrolled fall hazard of approximately 10 feet.
