29 CFR 1910.1030(d)(2)(i): Engineering and work practice controls were not used to eliminate or minimize employee exposure: (a) Sava Senior Care DBA Windsor Healthcare Center: On or before 11/28/12, the employer did not ensure that engineering and work practice controls were used to eliminate or minimize employee exposure. Employees were given Avanox prefilled syringe needles without a safety device. A safety device from another needle could have been used in conjunction with the Avanox needle. This condition exposed employees to a bloodborne pathogens hazard.

29 CFR 1910.1030(d)(2)(vii)(A): Contaminated needles and other contaminated sharps were bent, recapped, or removed: (a) Sava Senior Care DBA Windsor Healthcare Center: On or before 11/28/12, the employer did not ensure that contaminated needles and other contaminated sharps were not bent, recapped, or removed. Employees were given Avanox prefilled syringe needles without a safety device. The employees recapped the needle to protect other residents and staff from the exposed needle. This condition exposed employees to a bloodborne pathogens hazard. Sava Senior Care was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Administration Standard, 29 CFR 1910.1030(d)(2)(vii)(A), which was contained in OSHA Inspection number 98260, Citation number 1, Item 1a issued on October 7, 2011, and became Final Order on October 27, 2011, with respect to a workplace located at 1637 29th Ave. Place, Greeley, CO 80634.

29 CFR 1910.1030(d)(4)(iii)(A)(2)(i): During use, containers for contaminated sharps were not easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found: (a) Sava Senior Care DBA Windsor Healthcare Center: On or before 11/28/12, the employer did not ensure that, during use, containers for contaminated sharps were easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found. Employees used syringes to inject Avanox, insulin, and used lancets to test blood sugar levels of residents of the long term care center. This activity was conducted in the resident rooms without a sharps container in the room. The nearest sharps container was located on the nursing cart outside the residents room or at the nurses station. The distance to a sharps container was approximately ten to fifty feet away. This condition exposed employees to a bloodborne pathogens hazard. Sava Senior Care was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Administration Standard, 29 CFR 1910.1030(d)(4)(iii)(A)(2)(i), which was contained in OSHA Inspection number 98260, Citation number 1, Item 1b issued on October 7, 2011, and became Final Order on October 27, 2011, with respect to a workplace located at 1637 29th Ave. Place, Greeley, CO 80634.
