29 CFR 1910.303(g)(1)(iii): At least one entrance of sufficient area was not provided to give access to the working space about electric equipment:     (a)  SSC Windsor Operating Company LLC, dba Windsor Healthcare Center, at 710 3rd Street, Windsor, CO: On and preceding 3/10/16, the employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment.  Employees performed duties within the laundry room at Windsor Healthcare Center.  Duties included shutting off power to laundry equipment to enable repairs.  The path to access an electrical breaker panel was blocked by washing machines.  This condition exposed employees to an electrical hazard.

29 CFR 1910.1030(c)(1)(iv)(B): The annual review and update of the Exposure Control Plan did not include annual consideration and implementation of appropriate commercially available and effective safer medical devices designed to eliminate or minimize occupational exposure:    (a)  SSC Windsor Operating Company LLC, dba Windsor Healthcare Center, at 710 3rd Street, Windsor, CO:  On and preceding 3/8/16, the employer did not ensure that the annual review and update of the Exposure Control Plan included annual consideration and implementation of appropriate commercially available and effective safer medical devices designed to eliminate or minimize occupational exposure.  Employees performed nursing and other healthcare duties at Windsor Healthcare Center, including but not limited to injection of medications using syringes with needles, blood sugar checks using lancets, and administration of medication using IV's.  The employer determined that employees were potentially exposed to blood or other potentially infectious materials (OPIM).  The employer's review of the Exposure Control Plan did not include consideration of commercially available and effective safer medical devices.  This condition exposed employees to a bloodborne pathogen hazard.

After use, the contaminated sharp was recapped prior to disposal into a sharps container.  The employer did not ensure that contaminated sharps were not recapped. The employer's training program did not contain an explanation of the manufacturer's recommended disinfection contact time.  This condition exposed employees to a bloodborne pathogen hazard.

The employer recorded the injury on the 2014 OSHA 300 Log as "other recordable case" and did not place a check mark in Column I nor enter the number of days of restricted duty.
