29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    On or about 28 November 2012, employees were preparing the front facade of a home under construction for stucco work where some employees were underneath others while they were working, exposing the employees to struck by hazards from falling tools, materials and debris.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure employees wear protective helmets when the hazard of falling tools, materials, or debris exists at the work site.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    On or about 28 November 2012, an employee was using a tubular-welded frame scaffold without the use of a ladder to access the second tier platform, exposing the employee to a fall hazard of approximately 16 feet and 9 inches.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure a scaffold ladder is utilized for accessing the scaffolding system.    Waterloo Plastering, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA Inspection# 312853963, Citation# 01, Item# 001 and was affirmed as a final order on 1/12/2010, with respect to a workplace located at 1338 South Main Street, Boerne, TX 78006.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about 28 November 2012, two employees were applying paper, foam and mesh wire to the outside wall of a home under construction in preparation for stucco finish while on a roof without fall protection, exposing the employees to a fall hazard of approximately 11 feet and 10 inches.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure fall protection is provided any time work is being performed above 6 feet.    Waterloo Plastering, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA Inspection# 312853963, Citation# 02, Item# 001 and was affirmed as a final order on 1/12/2010, with respect to a workplace located at 1338 South Main Street, Boerne, TX 78006.
