29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Systech Handling, Inc., Archbold, Ohio: On or about January 28, 2012, employees operating the Willis Machinery & Tools manufactured CNC lathe (Model 2060ENC, Serial #0436) were exposed to injuries from the machine's rotating parts and flying particles, in that the interlock on the sliding door guard was bypassed. Employees were able to access hazard areas while the machine was in operation. b) Systech Handling, Inc., Archbold, Ohio: On or about November 28, 2012, the employer did not ensure that the rotating cutting blade was guarded on the MYD-MECH manufactured (Model S-20) band saw, in that an unused portion of the blade on the operators side was unguarded, exposing employees to amputation hazards. c) Systech Handling, Inc., Archbold, Ohio: On or about November 29, 2012, the employer did not assure that employees were protected from caught-by hazards while using the OOYA Machinery Works Co. LTD manufactured radial drill with an unguarded chuck to drill holes into stock.
