29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On 11/29/2012, the EDM (wire room) roof was being used for storage and hvac equipment. The open sided edge of the roof was 11 feet above the ground floor and approximately 180 feet in length. Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On 11/29/2012, the Amada horizontal bandsaws in the forge department saw area had unused portions of the rotating saw blades that were not guarded including rotating chip augers that were not guarded. PRESRITE CORPORATION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.212 (a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315312710, CITATION NUMBER 1, ITEM 3, AND WAS AFFIRMED AS A FINAL ORDER ON 6/21/2011, WITH RESPECT TO A WORKPLACE LOCATED AT 3665 EAST 78TH STREET, CLEVELAND, OHIO 44105.
