29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    a) Gerald Grain Center, Inc. - Archbold, Ohio: On or about November 29, 2012, the employer failed to provide medical evaluations to those employees who voluntarily wore half mask tight-fitting respirators while working in the mill warehouse. Employees were exposed to hazards from aggravation of pulmonary issues while wearing the respirators without medical clearance.

Employees were exposed to electrical shock and arc flash hazards.    b) Gerald Grain Center, Inc. - Archbold, Ohio: On or about September 20, 2012, the employer failed to ensure that employees used electrical safety related work practices when changing out 3 heaters in a 480V electrical cabinet for the NW Feed Auger, in that proper PPE was not worn and zero energy was not verified.
