29 CFR 1910.179(i):     Crane(s) equipped with a power traveling mechanism were not provided with a gong or other effective warning signal:      On or about November 29, 2012, and at times prior to, in the steel storage area an overhead crane was operated by a remote control device to lift heavy steel parts and was not provided with the means (audible alarm or strobe light) to warn employees working in the areas when the cranes were on the move.  Employees were exposed to struck-by hazards.

29 CFR 1910.106(e)(6)(ii):      Class I flammable liquid(s) were dispensed into containers, vehicles, and equipment without electrically interconnecting the nozzle and the container, vehicles, or equipment:      On or about November 29, 2012, and at times prior thereto, in the paint and flammable storage areas lacquer thinner Anchor J5 having flashpoint of less than 0 degrees F was dispensed into a smaller container without being effectively grounded and bonded to electrically interconnect the containers to prevent static discharge during dispensing operation exposing employees to a fire hazard.

29 CFR 1910.107(g)(5):     The use of solvents for cleaning operations was not restricted to those having flashpoints not less than 100 degrees F or  solvents having flashpoints not less than those normally used in the spray operation:      On or about November 29, 2012, and at times prior thereto, in the spray paint areas employees were using Anchor J5 lacquer thinner having flashpoint of less than 0 degrees F as a solvent for cleaning operations.  This exposes employees to fire hazards.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      On or about November 29, 2012, and at times prior thereto, a press brake was used without machine guards provided to prevent contact with the point-of-operation.  Employees were exposed to an amputation caused by the unguarded point-of-operation.
