Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a) Upper roof level: Employees without any means of fall protection were engaged in roofing work and exposed to a fall hazard of approximately 20 feet onto an asphalt driveway, on or about 11/28/2012.
