The Sperian N95 respirator used by employees was not approved to protect against lead based dusts.

Employees were not formally trained on the requirements of the respiratory protection standard for the safe use and limitations of the Sperian N95 respirator used during work operations.

The employees proceeded to remove the paint coatings from the king posts using needle guns; exposing themselves to lead containing dust created by the removal practices. On 11/30/2012, the work location was not free of lead containing dusts as evidenced through environmental wipe sampling.

Upon completion of the work activities, the lead containing chips and materials were dry swept and removed from the location; exposing employees to the hazards of lead containing dust.
