29 CFR 1910.263(f)(3): A divider was not provided with limit switches that prevented machine operation while the covers were open: (a) Rudi's Organic Bakery, Inc. at 3300 Walnut Street, Unit C, Boulder, CO 80301: The employer did not ensure that limit switches on a divider stopped the machine from operating. On November 28, 2012 the operator on Divider #2 was injured after opening the top rear cover of the equipment and reaching inside the machine. This condition exposed employees to injury by coming in contact with moving parts in the divider when the covers are opened.
