29 CFR 1926.651(b)(3): When excavation operations approach the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: a) Near the intersection of Fifth and Beachy Streets in Lewiston, ID; on or before November 26, 2012, and at times prior thereto, the employer did not anticipate the change in line depth and utilize hand digging or another safe means of locating an existing natural gas line prior to striking it with an excavator bucket. Note: Abatement certification and documentation ARE required for this item.
