29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) Located along the central interior area of the steel structure demolition operation employees were observed hand wrecking structural roof line purlins at varing elevations on or within a forklift mast attached raised safety platform exposing employee(s) working lower elevations to an above struck by hazard without utilization of head protective devices such as hard hat(s) / protective helmet(s).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. a) Located along the central interior area of the steel structure demolition operation a forklift was observed in use by employees working on or within a mast attached, safety platform with inadequate guardrail protection or equivalent. One employee is working while standing on top of the platforms guardrail system and a second employee worked from within the platform which was not provided with mid-rail protection on one open side exposing employees not otherwise equivaently protected to fall potentials in excess of 11'.
