Highland Avenue Orlando, FL: employees were exposed to stuck-by and caught in hazards in that employees were permitted to operate a Hercules 8 inch bench grinder Model HEO511 with a work rest adjusted to approximately one inch from the wheel, increasing the potential for a part to become lodged against the wheel during grinding.
