29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: (a) On or about November 8th 2012, a trailer located on the intake deck of The Dalles Dam had metal stairs used to access the S.E. side of the trailer. The stairs, which had seven risers, were not equipped with standard stair railings or standard handrails. (b) On or about November 8th 2012, a trailer located on the intake deck of The Dalles Dam had metal stairs used to access the N.W. side of the trailer. The stairs, which had four risers, were not equipped with standard stair railings or standard handrails. (c) On or about November 8th 2012, a trailer located on the intake deck of The Dalles Dam had metal stairs used to access the S.W. side of the trailer. The stairs, which had four risers, were not equipped with standard stair railings or standard handrails.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): The Dalles Dam, intake deck- (a) The pulleys of the heat tank vacuum pump and motor were not guarded, exposing employees to hazard of the rotating pulleys. The unit was located at the floor level of the trailer.
