29 CFR 1926.602(c)(1)(vi):     Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.    (a)  South Berkeley Recreation Center:  Employees operated a Kalmar AC forklift (model #C5OBL PS, serial #406089A) that was equipped with inoperable back up alarm.  The forklift was not taken out of service until the back up alarm was restored to safe operating condition, as determined on December 4, 2012.    Abatement Note:  American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks, Section 702, Paragraph I states, "Brakes, steering mechanisms, control mechanisms, warning devices, lights, governors, lift overload devices shall be inspected regularly and maintained in safe operating condition."
