29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about November 21, 2012, in the front of the building at 4775 Loma Del Sur Dr. in El Paso, two employees were working on a scaffold, without any form of fall protection. This condition exposed the employee to a fall hazard of approximately 12 feet to ground level.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: Construction site: On or about December 6, 2012, at the entrance of the parking lot located at 4775 Loma Del Sur Dr.; protruding steel form rods were not guarded to eliminate the hazard of impalement. This hazard exposed employees to an impalement hazard of approximately 12 inches.
