29 CFR 1926.451(b)(1): A platform on a working level of scaffold was not fully planked or decked between the front uprights and the guardrail supports.    On or about May 30, 2013 and times prior thereto, two employees were working on a platform that was erected at a height more than 10 feet from the ground that was not fully planked between uprights and guardrail supports.  This condition exposed employees to fall hazards greater than 10 feet from the ground.

On or about May 30, 2013, and times prior thereto, two employee were working on a scaffold platform not fully planked with heights more than 10 feet from the ground without any type of fall protection system, and erected right by protruding reinforced steel bars that were not guarded. These conditions expose employees to impalement hazards from falling.

On or about May 30, 2013, and times prior thereto, two employees were using a secured portable extension ladder to access the residence roof with heights more than 15 feet from the ground that was not extend at least 3 feet (.9 m) above the upper landing surface, and was not provided with grab rail.  These conditions exposed employees to fall hazards.
