29 CFR 1915.74(b): Gangways meeting the requirements of 29 CFR 1915.24(a)(1), (2), (9), (10), and (11) were not provided for access from wingwall to vessel, or were not provided when two or more vessels, other than barges or river towboats, were lying abreast. At the time of the inspection, multiple instances of missing top rails and mid rails were found on access areas on dry docks #1, #2, #3, #4, and top side repair location.

29 CFR 1915.89(d)(1): The employer shall establish and implement written procedures to prevent energization or startup, or the release of hazardous energy, during the servicing of any machinery, equipment, or system. At the time of the inspection, the employer had not developed written procedures for equipment such as but not limited to the plasma cutter and hydraulic shear.
