29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On 06/06/2017, employees working at JB Marine Service, Inc. located at 4190 Bussen Rd., St. Louis, MO, 63129, were exposed to a forklift in an unsafe condition. The Tusk Lift truck (Model: 300CGH-20, S/N: 677016A) had its LP tank improperly mounted. The tanks slot for receiving the locator/index pin for proper mounting was not properly mounted over the pin.
