(b) The employer did not ensure that each employee working on the Die Cutter #1 (PMC Die Cutting Machine Series FMA 73 Serial FMA 873) was protected from amputation hazards in that two employees worked at the machine with only one employee being protected by the use of a two hand control. (c) The employer did not ensure that each employee working on the Die Cutter #2 (PMC Die Cutting Machine, Series B, Serial #AB40) was protected from amputation hazards in that two employees worked at the machine with only one employee being protected by the use of a two hand control. (d) The employer did not ensure that each employee working on the Die Cutter #3 (PMC Die Cutting Machine, Model #L, Serial #L-1381) was protected from amputation hazards in that two employees worked at the machine with only one employee being protected by the use of a two hand control.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) A drop down plug had its outer sheath pulled away due to lack of strain relief. This exposes any employee using it to shock hazards and also all employees to fire hazards.
